New Client Information and Registration


At the first visit with us, we ask clients to complete a New Client Registration Form prior to your visit. If you do choose to complete the registration form in advance, please  print and bring it to your first appointment.

We also request that new clients complete the appropriate Pet History Form for each of their pet(s) to provide us with a complete history. You may click on any of the following links to obtain the appropriate form for your pet(s) in advance of your first visit.

Avian Friends

All new bird clients and annual wellness visits are encouraged to bring several photographs of the environment, caging and diet so that the doctor can better understand how your pet is housed and cared for at home.

Canine & Feline Friends

Please bring a fresh stool sample (within 24 hours) to your first visit and to each annual wellness visit. Intestinal parasites can be transmitted from pet to pet and from pets to people, so it is extremely important for your health as well.  If your dog or cat is experiencing problems with urination or defecation, it is also helpful to bring a sample of the urine or feces with you.

Ferret, Rabbit, Rodent & Reptile Friends

At the time of your appointment, please bring in a fresh stool sample as well as a picture or two of your pet's cage set up and diet. This will help the Doctor to better understand the environment that your pet lives in.
